VITALITY ATTENDS HISPANIC FAMILY CENTER OF NEW JERSEY’S ANNUAL HEALTH FAIR

On June 10, 2016, representatives from Vitality Catholic Healthcare Services attended the Hispanic Family Center of New Jersey’s Annual Community Health Fair and Concert, at 29th and Federal Streets in Camden.

Mimi Schaible, RN, MSN, Director of Care Coordination & Consultation and Natasha Sauls, Call Center Coordinator, provided literature in both Spanish and English about Vitality’s recently developed Resource and Referral Help Line and Care Coordination Programs.

Over 300 individuals and families visited Vitality’s information table. Many talked about their loved ones, mostly seniors and persons with disabilities, who need help at home.

Natasha, who is bilingual, answers the telephone for Vitality’s Resource and Referral Help Line and provides contact information for community and social services. She also arranges for Vitality’s Care Coordinators, Nurses and Social Workers, to visit individuals in the home to connect them with needed medical and community services. Since the program began in December, 2015, Vitality’s Help Line operators and Care Coordinators have helped connect over 300 seniors and persons with disabilities with home and community services.
